Effective 10/09/2014. Dr Smith will no longer be seeing patients in the Optical Shop and Showroom.

Due to the pending potential sale of the building that The Optical Shop and Showroom occupies, Dr. Smith will operating at this location. 

It was a pleasure helping clients in the north Tampa area, and you are more than welcome to visit our South Tampa office to continue your care with Dr. Smith. If any patient would like copies of records, please send a signed statement for the release of your information and also include a copy of a photo ID.
